 چکیده: این مقاله مروری است بر سه فرضیه متفاوت که به بررسی دلایل گرایش مردم به طب مکمل می پردازد. براساس فرضیه های مبتنی بر نتایج، دلیل گرایش به درمان های طب مکمل، نارضایتی مردم از نتایج درمان های طب رایج است. براساس فرضیه های مبتنی بر برخوردارهای پزشکان، دلیل این گرایش نارضایتی مردم از الگوهای کنش و تعامل پزشک-بیمار است. براساس فرضیه های"پست مدرن" دلیل گرایش مردم به طب مکمل، ظهور یک نظام
ارزشی جدید متناسب با محتوای فلسفی طب جایگزین است. تحلیل های چند متغیره نتایج براساس یک نمونه تصادفی، فرضیه های برخورد پزشکی و پست مدرن را تأیید می کند و نشان می دهد که مجموعه ای از ارزش های پست مدرن در مقایسه با نارضایتی از برخوردهای پزشکی، پیش بینی کننده بهتری برای تبیین گرایش به درمان های طب مکمل اند.
مقدمه: شواهد بسیاری بر رشد سریع و گسترده استفاده از درمان های طب مکمل در استرالیا، نیوزلند، ایالات متحده آمریکا و بسیاری از کشورهای اروپایی دیده می شود. پژوهش های زیادی تبیین های نظری بسیاری را برای گرایش به درمان های طب مکمل بیان می کنند. این تبیین ها، سه عامل مؤثر را در این راستا به شرح زیر شناسایی می کنند: نارضایتی از نتایج حاصل از درمان های طب رایج، نارضایتی از الگوی روابط حاکم بر پزشک–بیمار، و ظهور یک سیستم ارزشی که تحت عنوان تئوری های پست مدرن شناخته می شوند. فقط تعداد کمی از این تحقیقات فراتر از نظریه پردازی ها و گمانه زنی ها تلاش کردند که علت روند رو به رشد گرایش به طب مکمل را به صورت تجربی بیان کنند. در میان این مطالعات، تنها در یک پژوهش، از روش تجزیه و تحلیل ساده دو متغیره استفاده شده است(یاتس و همکاران1، 1993). تا کنون، هیچ مطالعه تجربی، تبیینی از تحلیل های چند متغیره براساس یک نمونه تصادفی از جامعه ارایه نکرده است. در این مقاله، ابتدا، تلاش می کنم مروری بر مطالعاتی که گرایش مردم به درمان های مکمل را توضیح می دهند؛ داشته باشم. سپس براساس یک نمونه تصادفی، یک مدل چند متغیره را که به صورت تجربی رابطه این تبیین ها را ارزیابی می کند، توسعه می دهم.
نارضایتی از نتایج درمان های طب رایج
این تبیینات بیانگر آن است که مردم به این دلیل به درمان های طب مکمل روی می آورند که از درمان های طب رایج که مدعی بود درد و رنج را ریشه کن می کند و سلامتی مطلوبی برای بشریت به ارمغان می آورد، مأیوس شده اند(هولدن2 ،1978). طب رایج قادر نیست بیماری های دژنراتیو و بیماری های مزمن مثل سرطان و بیماری های قلبی را درمان کند، همچنین قادر نیست دردهای ناشی از کمردرد، گردن درد، آرتریت و رماتیسم را کاهش دهد(آنینام3 1990، انگلیس و وست4 1983). به علاوه، در سال های اخیر، شاهد رشد فزاینده شمار زیادی از بیماری های ایاتروژنیک به علت عوارض دارویی و تجهیرات دارویی مانند آنتی بیوتیک ها و داروهای ضد افسردگی هستیم.(برای مثال نگاه کنید به آنینام،1990)
شارما5(1992) مستنداتی را در تایید این فرضیه گزارش کرد که بیماران، اساسأ به خاطر آن که برای درمان بیماریشان- که در طب رایج علاجی برای آن نبوده- راهی بیابند، به روش های درمانی طب مکمل گرایش پیدا می کنند. به همین ترتیب، فورنهام و اسمیت6(1988) و فورنهام و فوریه7(1994)، بیماران مراجعه کننده به طب رایج و مکمل طب مکمل را با هم مقایسه کردند و نشان دادند که گروه دوم(مراجعه کنندگان به طب مکمل)، نسبت به تأثیر درمان های طب رایج شکاکیت بیشتری دارند. این نتیجه گیری براساس سنجش پاسخ هایی که به اظهار نظرهایی مانند "پزشکان فقط قادرند برخی از بیماران را درمان کنند" و"اغلب مردم زمانی که واقعأ چاره ای ندارند، نزد دکتر می روند." به دست آمده است.
نارضایتی از برخوردهای پزشکی
در تأیید این استدلال که ادعا می کند مردم به دلیل آن که از نتایج درمان های طب رایج، ناراضی اند به طب مکمل روی می آورند؛ این فرضیه مطرح می شود که آنان به طور کلی از نحوه برخورد پزشکان با بیماران نیز ناراضی اند. ( ایستهوپ8 1993؛ شارما 1990؛ تایلر9 1984؛ پارکر و تاپلینگ10 1977).در نهایت این استدلال فرض را بر این می گیرد که پزشکان "احساسات انسانی" خود را دست داده اند. آنها وقت بسیار کمی برای بیمارانشان می گذارند، برای آنان احترام زیادی قایل نیستند و اطلاعات کافی درباره ماهیت بیماری، تشخیص و شیوه درمان به بیمارانشان نمی دهند. پزشکان یک ایدئولوژی ماتریالیستی را به جای یک ایدئولوژی خدمت- محور اتخاذ می کنند و در رابطه با بیمار، به جای آن که از یک الگوی مشارکتی استفاده کنند، از الگوی پزشک- محور تبعیت می کنند. پزشکی امروز، چنانچه شاید به بهترین نحو توصیف شده است یک پزشکی فوردیستی است که به کمیت بیش از کیفیت توجه دارد و این امر، بیگانگی و نارضایتی بیمار را به همراه دارد. شواهد بسیاری در تأیید این تبیین در تحقیقات موجود است. مصاحبه های شارما(1992) با مراجعه کنندگان به درمانگران طب مکمل نشان می دهد که آنها معتقدند که پزشکان طب رایج، زمان بسیار کمی را به بیماران اختصاص می دهند. فورنهام و فوریه(1994) همچنین دریافتند مراجعه کنندگان به درمانهای طب مکمل، اعتقاد دارند که پزشکان طب رایج به اظهارات بیمارانشان توجه نمی کنند.
ظهور ارزش های پست مدرن
بر مبنای این نظریه در اواخر دوره مدرن، یک سیستم ارزشی ظهور پیدا کرد که اعتقادات جدیدی را درباره طبیعت، علم و تکنولوژی، سلامتی، قدرت، مسئولیت فردی و مصرف گرایی ارایه می کند. از این پس، از این ارزش ها تحت نام ارزش های پست مدرنی یاد می کنم( نگاه کنید به ایستهوپ 1993؛ بکث11 1991). به نظر می رسد که تناسب این ایده ها با فلسفه درمان های مکمل، باعث محبوبیت و گسترش این روش های درمانی شده است. پزشکان طب مکمل معمولأ از درمان های طبیعی و غیرتهاجمی استفاده می کنند، دیدگاهی کل نگر نسبت به سلامتی دارند، به بیمارشان اجازه می دهند که درروند درمان مشارکت داشته باشد( برای مثال نگاه کنید به اکستر12 1989)، و تاکید می کنند که سلامتی ذاتا امری فردی است و نهایت، فرد برای رسیدن به وضعیت مطلوب سلامت، مسئول است. (کووارد13، 1989)
کووارد اذعان می کند در حالی که در گذشته ، همچنان که در آثار روسو منعکس شده، طبیعت با مفاهیمی مثل توحش، تجاوز، خشونت و بقای اصلح که بیانگر یک جامعه بدوی است همراه بوده؛ امروزه ما طبیعت را بسیار ملایم، مهربان، خیرخواه و امن می یابیم. ما اهمیت می دهیم که محصولات طبیعی مصرف کنیم، و سعی می کنیم که از مصرف محصولات غذایی آماده که دارای انواع نگه دارنده ها و رنگها هستند اجتناب کنیم. این ارزش همچنین نظرات بکث(1991) را بازتاب می دهد که ادعا می کند پست مدرن ها به یک "فرهنگ سبز" معتقدند و بر اساس آن، بدن انسان را به مثابه گسترده ای از طبیعت معنا می کنند و معتقدند که خوردن داروهای شیمیایی درحکم آلوده کردن طبیعت با زباله های شیمیایی است.
از این دیدگاه، همان طور که طبیعت ملایم و مهربان است؛ به همان ترتیب دستاوردهای علم و تکنولوژی ، بالقوه مضر و خطرناک به نظر می رسند. بهره گیری از تکنولوژی در درمان به مثابه نوعی دخالت و تغییر طبیعت و به وجود آوردن حالت های تهاجمی در روند درمان بیمار است( کووارد،1989). کرز14 (1994) استدلال می کند ما شاهد رشد فزاینده ضدیت با علم و به وجود آمدن سوءظن هایی در زمینه تحقیقات زیست پزشکی هستیم. (همچنین نگاه کنید به پارک 15 ، 1996). برخی پیش بینی می کنند که این گرایش در آینده ای نزدیک، قوی تر و گسترده تر خواهد شد. ( نیکلسون 16، 1993)
برخی مستندات تجربی بر وجود رابطه بین استفاده از طب مکمل و تعهد به فرهنگ استفاده از محصولات طبیعی وجود دارد. شارما (1992) نشان داد که افرادی که درمان های طب مکمل را انتخاب می کنند نسبت به عوارض جانبی داروهای شیمیایی آگاهی دارند و فکر می کنند که درمانهای طب رایج بسیار تهاجمی و خشن هستند. همچنین فورنهام و فوریه(1994) گزارش کردند که بیمارانی که از طب مکمل استفاده می کنند به احتمال زیادتری از "افزودنی ها و نگهدارنده ها" در رژیم غذایی شان استفاده نمی کنند، از "مغازه هایی که غذایی سالم می فروشند" خرید می کنند، و از مصرف "داروهای شیمیایی تجویز شده" خودداری می کنند.
از سویی اعتقاد ما درباره سلامتی نیز تغییر کرده است. آنینام (1990) بیان می کند یک تغییر اساسی به سوی درمان های کل نگر درحوزه سلامت انجام گرفته است. امروزه مردم به سلامتی به عنوان یک عنصر چند وجهی شامل بدن، ذهن و روان نگاه می کنند. درواقع هماهنگی بین این موارد- بدن، ذهن و روان- پیش نیاز برخورداری از سلامتی کامل است. سلامتی به طور ذاتی در درون ما وجود دارد و امری نیست که در اثر مصرف داروها به دست بیاید( نگاه کنید به کووارد،1989). فورنهام و اسمیت (687:1988) در تایید این مطالب، مستنداتی را آورده اند که بیانگر رابطه این ارزش ها درباره سلامتی و استفاده از طب مکمل است. آنها نشان دادند که بیمارانی که از طب مکمل استفاده می کنند کمتر به این موضوع اعتقاد دارند که " درمان بایستی به روی نشانه های بیماری متمرکز باشد تا به روی کل بدن"، و بیشتر به این موضوع باور دارند که "بدن توانایی درمان خود را دارد" (همچنین نگاه کنید به فورنهام و فوریه، 1994).
بی اعتنایی عمومی به اقتدار ، به ویژه اقتدار حرفه ای و علمی و تقاضا برای مشارکت در روند درمان از دیگر عواملی است که به رشد درمان های طب مکمل کمک کرده است( رایزمن17 1994، ایستهوپ 1993،تایلر 1984). بیماران مایلند که در روند درمان مشارکت داشته باشند و بیش از این حاضر نیستند تفاوتی را که در میزان اقتدار آنها و پزشکانشان وجود دارد، قبول کنند. مردم علاقمندند که مشارکت بیشتری در مشاوره پزشکی داشته باشند و می خواهند که شانس مشارکت در درمان خود را داشته باشند(کارول18 1994). در تأیید وجود رابطه بین استفاده از طب مکمل و نگرش نسبت به قدرت، ییتس و همکارانش( 205:1993) نشان دادند که کسانی که از این نوع خدمات استفاده می کنند احتمالا بیشتر " نیاز به کنترل تصمیمات اتخاذ شده در مورد درمان دارند" و آنها "نمی خواهند اختیارشان را برای تصمیم گیری در مورد درمان به پزشکشان واگذار کنند". همچنین فورنهام و اسمیت(687:1988) نشان دادند بیماران طب مکمل، در مقایسه با بیماران استفاده کننده از طب رایج، کمتر مایلند سلامتی شان را در دستان حرفه ای ها بگذارند.
توجه به مسئولیت های فردی و برتری افراد بر این اساس یکی از مهمترین شاخصه های اواخر دوران مدرن است(کسیلیث19 1989:248). مردم بیشتر تمایل به پذیرش این امر دارند که در نهایت "افراد" خودشان مسئول سلامتی شان هستند و برای دستیابی به سلامتی نیازمند کار سخت، تعهد و تغییر شیوه زندگی اند(کوارد1989). اخیرا به طور واضح در رسانه های عمومی بر رژیم غذایی، ورزش، استراتژی های کمک به خود، مدیریت استرس و تغییر شیوه زندگی تأکید زیادی می شود. یک کار تجربی از فورنهام و فوریه(1994) بر وجود رابطه بین باور به مسئولیت های فردی و استفاده از طب مکمل تأکید می کند. آنها نشان دادند که بهره گیرندگان از درمان های طب مکمل کمتر با این عبارت که"کمتر کسی است که بتواند از بروز بیماری پیشگیری کند" موافقند و بیشتر با این عبارت موافقند که" در دراز مدت افرادی که از خودشان مراقبت می کنند سالم باقی می مانند یا سریع تر خوب می شوند."
جنبش حمایت از مصرف کنندگان، یکی دیگر از دلایل گرایش مردم به درمان های طب مکمل است که طیف وسیعی از انتخابها را پیش روی مردم می گذارد.(رایزمن 1994؛ ایستهوپ 1993؛ بکث 1993؛ کسیلیث 1989). امروزه مردم در راستای بهره مندی و رضایت بیشتر به راحتی عادات مصرفی شان را تغییر می دهند. خیلی واضح به چشم می خورد که در زمینه بهداشت، مفاهیم حمایت از مصرف کنندگان، فعالیت های سیاسی و حقوق بیماران، به بخشی از واژگان عموم تبدیل شده اند(کارول 1994). تا کنون رابطه بین گرایش مصرف کنندگان و تمایل به استفاده از درمان های طب مکمل اساسأ مورد آزمایش قرار نگرفته است.
فرضیات: براساس مطالب فوق، این مقاله وجود رابطه بین فرضیات زیر را بررسی می کند.
ÿ فرضیه نتایج پزشکی: افرادی که از نتایج درمان های طب رایج ناراضی هستند احتمالا بیشتر به درمان های طب مکمل گرایش پیدا می کنند.
ÿ فرضیه برخورد های پزشکی: افرادی که برخوردهای پزشکی در طب رایج ناراضی هستند احتمالا بیشتر به استفاده از درمان های طب مکمل گرایش دارند.
ÿ فرضیه پست مدرنی: افرادی که دارای دیدگاه های پست مدرنی هستند بیشتر به استفاده از درمان های طب مکمل گرایش دارند.
نمونه آماری: اطلاعات بدست آمده از این تحقیق براساس یک مصاحبه تلفنی از 209 نفر از بزرگسالانی که در منطقه های با کد 060 در استرالیا زندگی می کنند، به دست آمده است.
نتایج بررسی نشان داد:
الف) نارضایتی از نتایج درمان های طب رایج: افراد با امتیاز بالاتر در این مقیاس بیشتر از نتایج درمانهای طب رایج ناراضی هستند. آنها اعتماد کمتری به توانایی پزشکان در درمان بیماران دارند.
ب) نارضایتی از برخوردهای پزشکی: افراد با امتیاز بالاتر در این مقیاس، بیشتر از رفتارهای پزشک با بیمار اظهار نارضایتی می کردند. آنها به شدت معتقدند که پزشکان به صحبت های بیمارانشان گوش نمی کنند؛ به آنها احترام نمی گذارند؛ به اندازه کافی برای آنها وقت نمی گذارند؛ و بسیار عامرانه با آنها صحبت می کنند.
ج) درمان های طبیعی: افراد با امتیاز بالاتر در این مقیاس، برای درمان های طبیعی ارزش زیادی قائل اند و نسبت به داروهای شیمیایی و نسخه های پزشکان بی اعتمادند.
د) احساسات ضدتکنولوژیکی: افراد با امتیاز بالاتر در این مقیاس باور دارند که پیشرفت های تکنولوژیکی محیط را برای زندگی انسانها مضر سازد.
ه) کل گرایی: افراد با امتیاز بالاتر در این مقیاس گرایشات کل گرایی دارند. آنها معتقدند که سلامتی نتیجه تعادل و سازگاری میان بدن، ذهن و روان است و درمان بایستی بر کل فرد متمرکز باشد.
و) طرد اقتدار پزشکی: افراد با امتیاز بالاتر در این مقیاس، اقتدار آمرانه پزشک را که از سوی آنها اعمال می شود طرد می کنند. این افراد معتقدند که پزشکان بایستی بیمارانشان را با نگاهی برابر درمان کنند و بیماران بایستی در روند درمان مشارکت داشته باشند.
ز) مسئولیت های فردی: افراد با امتیاز بالا در این مقیاس بر نقش مسئولیت های فردی در حفظ سلامتی تأکید می کنند. آنها معتقدند دستیابی به سلامتی نیازمند تعهد و تغییر سبک زندگی است و این ما هستیم که انتخاب می کنیم.
س) مصرف گرایی: افراد با امتیاز بالاتر در این مقیاس گرایشات مصرف گرایانه دارند و از تنوع انتخاب در درمان استقبال می کنند.
بحث و نتیجه گیری: اهمیت این تحقیق در رشته نوپای جامعه شناسی طب مکمل در ماهیت تئوری مند بودن و چند متغیره بودن آن است. براساس پاسخ های ارایه شده از یک نمونه تصادفی از افراد به مجموعه ای از سوالات، توانستم سه فرضیه متفاوت را که علل و گرایش مردم به درمان های طب مکمل را تبیین می کرد بررسی کنم: نتایج حاصل از نارضایتی از درمانهای طب رایج، برخورد پزشکی و ارزش های پست مدرنی نشان داد که دو فرضیه اخیر در موضع خود پا برجا هستند و ظهور ارزش های پست مدرنی در مقایسه با نارضایتی از برخوردهای پزشکی تبیین بهتری از گرایش مردم به درمان های طب مکمل ارایه می کند. سه نتیجه اصلی را می توان از نتایج این تحقیق به دست آورد. اول، متغیرهای جمعیت شناختی، جنس، سن و تحصیلات به روی گرایش به درمانهای طب مکمل ندارند و فقط می تواند به میزان کمی این متغیرها را تبیین کند. حتی در سطوح دو متغیره، این متغیرها بر گرایش به درمان های طبیعی تأثیری ندارند، و گرایش به درمان های طب مکمل در میان سنین و گروه های مختلف جمعیت شناسی به میزان یکسانی در بین مردان و زنان وجود دارد.
دوم، نارضایتی از نتایج درمان های طب رایج تأثیر قابل ملاحظه ای بر گرایش مردم به استفاده از درمان های طب مکمل ندارد. در حالی که زمانی که نارضایتی از نتایج درمان های طب رایج با نارضایتی از برخوردهای پزشکی توام می شود، گرایش قابل ملاحظه ای را به استفاده از درمان های طب مکمل به وجود می آورد. مردم از درمان های طب رایج رویگردان شده اند نه تنها به این علت که طب رایج در ارایه سلامتی به آنها ناکام بوده است بلکه به علت رفتار و برخورد پزشکان با بیماران از آنها رویگردان شده اند. این موضوع درست و جدای از نظام ارزش های افراد است، زیرا تأثیر تجربه پزشکی فراتر از ارزش های پست مدرن است.
مردم به استفاده از درمان های طب مکمل گرایش دارند برای این که فکر نمی کنند پزشکان احترام کمی برای آنان قائلند، برای آنان وقت کافی نمی گذارند، به آنها گوش نمی کنند، بسیار آمرانه با آنان رفتار می کنند و به آنان فرصت مداخله در روند درمان نمی دهند. به طور خلاصه، کیفیت رابطه بین پزشک – بیمار و تفاوت در میزان اقتدار در رابطه فی مابین پزشک و بیمار، بخشی از دلایل گرایش عمومی به درمان های طب مکمل است.
سوم، و مهمترین دلیل، که این تحقیق نشان داده است برخورداری از ارزش های پست مدرنی اصلی ترین تبیین برای گرایش مردم به استفاده از درمان های طب مکمل است. افرادی که به درمان های طبیعی اعتقاد دارند، باوری به نسخه های پزشکان که داروهای شیمیایی را تجویز می کنند ندارند، معتقدند که تکنولوژی، محیط زندگی را برای مردم مضر ساخته است و مردم را به استفاده از درمان های طب مکمل سوق داده است. به این ترتیب یافته های این تحقیق از نظریه های تئوریسین هایی که به ظهور ارزش های پست مدرنی تأکید می کنند و آن را تبیینی برای ظهور طب مکمل می دانند حمایت می کند حمایت می کند. به نظر می رسد که این فلسفه و سیستم ارزشی رو به افول نیست و برعکس، فرهنگ سبز(فرهنگ حامی استفاده از طبیعت) بر مصرف غذاهای طبیعی تأکید می کند. جنبش حمایت از مصرف کنندگان و آگاهی از مضرات تکنولوژی خیلی گسترده شده است و به این ترتیب انتظار می رود که در آینده محبوبیت و استفاده از درمان های طبیعی رشد بیشتری پیدا کند.
